One of the most common claims is that pharmaceutical companies have little incentive to seek inexpensive cures because it is more profitable to keep patients on long- term treatment than to cure them outright.
The core criticism is that the pharmaceutical industry's business model rewards long-term treatments rather than definitive cures.
The reasoning is as follows:
A patient who takes a medicine for 20 years generates substantially more revenue than one who is cured after a single treatment.
Pharmaceutical companies are profit-making businesses that are accountable to shareholders.
Consequently, they may have stronger incentives to invest in chronic conditions than in permanent cures.
The strongest accusation: "companies prefer chronic patients to cured patients".

Los autores recopilaron, organizaron y analizaron 91 ensayos clínicos de ivermectina, un compuesto antiinfeccioso macrólido, contra la infección por el nuevo coronavirus (SARS-CoV-2) (COVID-19). Estos ensayos clínicos se registraron en organismos públicos de registro y se llevaron a cabo en 27 países de todo el mundo hasta marzo de 2021. Los resultados se presentan en una revisión en japonés e inglés publicada en esta revista.

Posteriormente, los ensayos clínicos de ivermectina para la COVID-19 continuaron aumentando a nivel mundial. En noviembre de 2023, dos años y ocho meses después, se habían registrado 169 ensayos clínicos en 39 países. Además, se publicaron 54 ensayos clínicos no registrados, lo que elevó el total a 223 ensayos clínicos, incluyendo los registrados. Se informaron los resultados de 101 de estos ensayos clínicos, junto con 27 revisiones sistemáticas y metaanálisis realizados sobre 69 artículos de resultados de ensayos clínicos de 24 países. Los autores resumieron y analizaron estos hallazgos, presentándolos en una revisión en japonés e inglés publicada en esta revista.

Aunque la COVID-19 se trató como una sola enfermedad infecciosa, el virus causante —SARS-CoV-2— es único y muta rápidamente, mostrando una alta variabilidad. Las cepas mutantes continuaron evolucionando con el tiempo y se manifestaron con diferente infectividad y patogenicidad. Por lo tanto, es apropiado considerar las etapas iniciales y posteriores de la pandemia mundial de COVID-19 como una enfermedad infecciosa con diferentes patologías inherentes. La cepa inicial de Wuhan tenía una infectividad limitada, pero una patogenicidad notablemente alta. Esto resultó en una alta tasa de mortalidad, debido a la inflamación pulmonar grave. Por otro lado, la variante Ómicron, que se propagó ampliamente por todo el mundo a partir de enero de 2022, fue altamente contagiosa y causó un número significativamente grande de casos. Sin embargo, con la variante Ómicron, los síntomas de la infección se limitaron principalmente a las vías respiratorias superiores, extendiéndose hasta el nivel de la laringe. GLOBAL TRENDS IN CLINICAL TRIALS OF IVERMECTIN FOR COVID-19 (2026)

The authors compiled, organized and analyzed 91 clinical trials of ivermectin, a macrolide anti-infective compound, against the novel coronavirus (SARS-CoV-2) infection (COVID-19). These clinical trials were registered with public registration agencies and conducted in 27 countries around the world as of March 2021, and the results were presented in a Japanese and English review published in this journal.

Subsequently, clinical trials of ivermectin for COVID-19 continued to increase worldwide. By November 2023, two years and eight months later, 169 clinical trials in 39 countries had been registered. An additional 54 unregistered clinical trials had been published, bringing the total overview to 223 clinical trials—including the registered trials. The results of 101 of these clinical trials were reported, along with 27 systematic reviews and meta-analyses conducted on 69 clinical trial results papers from 24 countries. The authors summarized and analyzed these findings, presenting them in a review in Japanese and English published in this journal.

Although COVID-19 was treated as a single infectious disease, the causative virus—SARS-CoV-2—is unique and mutates rapidly, exhibiting high variability. Mutant strains continued to evolve over time and manifest with different infectivity and pathogenicity. Therefore, it is appropriate to consider the early and later stages of the global pandemic of COVID-19 as an infectious disease with different inherent pathologies. The initial Wuhan strain had limited infectivity, but remarkably high pathogenicity. This resulted in a high mortality rate, due to severe lung inflammation. On the other hand, the Omicron variant, which spread widely around the world from around January of 2022, was highly contagious and caused a significantly large number of cases. With the Omicron variant, however, infection symptoms were primarily limited to the upper respiratory tract—extending to the level of the larynx. Severe lower respiratory tract infections involving the bronchi and lungs rarely occurred. This denoted a weak inherent pathogenicity. Such changes in the pathogenicity of COVID-19 over time are a significant variable behind the conduct and progression of clinical trials involving ivermectin. The authors investigated the global trends of the SARS-CoV-2 mutant strains—up to that time—and published an additional review in this journal in Japanese and English, along with the development status of therapeutic drugs.

This review will organize and analyze clinical trial results that were not available at the time of writing the three above-mentioned reviews, and results that have since been published. We will analyze the outcomes of these clinical trials, as well as summarize and discuss the results of basic research that forms the basis for clinical trials and general discussions.

Klebsiella pneumoniae (Kpn) es una de las principales causas de sepsis neonatal en África. La resistencia a las cefalosporinas de tercera generación y a la gentamicina por parte de Kpn es común en muchos lugares, lo que hace que los antimicrobianos de primera y segunda línea recomendados por la OMS sean ineficaces. Comprender qué neonatos y lactantes tienen mayor riesgo de sepsis causada por Kpn respaldaría la necesidad de mejorar el acceso a los antimicrobianos de vigilancia y reserva de la OMS (por ejemplo, carbapenémicos) para los pacientes que más se beneficien de ellos.

Se realizó un estudio prospectivo de casos y controles en el Hospital Central Queen Elizabeth de Malawi. Los casos fueron lactantes menores de 3 meses con infección por Kpn confirmada mediante cultivo de sangre o líquido cefalorraquídeo. Los controles fueron lactantes sanos de las mismas salas y se emparejaron en una proporción de 2:1. Se realizó una regresión logística univariante y multivariante sobre datos centrados en la media para determinar los factores de riesgo de infección por Kpn.

Analizamos datos de 38 casos y 76 controles entre [insertar fechas]. La mortalidad a los 3 meses de edad fue de 21/38 (29%) para los casos, con 14/38 (37%) identificados post mortem, y 6/76 (7,9%) para los controles (OR 14,0 (IC del 95%: 4,59–49,2, p>0,001). Los casos tenían más probabilidades de nacer fuera del QECH que los controles (42% frente a 24%, p=0,043), y los casos tenían pesos al nacer más bajos (mediana 2200g frente a 2850 g, p = 0,005). El análisis de regresión logística multivariante reveló que el aumento del peso al nacer era protector contra la infección por Kpn (OR: 0,858 [IC del 95%: 0,745–0,987] por cada aumento de 100 g), mientras que una estancia hospitalaria más prolongada se asoció con mayores probabilidades de infección (OR: 1,148 [IC del 95%: 1,012–1.1.303] por día adicional). La mayoría de los aislados infecciosos (34/38 [89%]) fueron resistentes a los agentes antimicrobianos de primera y segunda línea, pero todos fueron sensibles al meropenem y 33/36 [92%] a la amikacina.

Klebsiella pneumoniae (Kpn) is a major cause of neonatal sepsis in Africa. 3rd-generation cephalosporin and gentamicin-resistant Kpn is the norm in many sites, rendering WHO-recommended first- and second-line antimicrobials ineffective. An understanding of which neonates and infants are most at risk of sepsis caused by Kpn would support the case for improved access to WHO Watch and Reserve antimicrobials (i.e. carbapenems) for patients most likely to benefit from them.

A prospective case-control study was conducted at Queen Elizabeth Central Hospital, Malawi. Cases were infants <3 months of age with blood or CSF culture-confirmed Kpn infection. Controls were healthy infants from the same wards and were matched 2:1. Univariate and multivariate logistic regression were performed on mean-centred data to determine risk factors for infection with Kpn.

We analysed data from 38 cases and 76 controls between [insert dates]. Mortality at 3 months of age was 21/38 (29%) for cases, with 14/38 (37%) identified postmortem and 6/76 (7.9%) for controls (OR 14.0 (95% CI 4.59, 49.2, p>0.001). Cases were more likely to be born out of QECH than controls (42% vs. 24%, p = 0.043), and cases had lower birthweights (median 2200g vs. 2850g, p = 0.005). Multivariate logistic regression analysis revealed that increasing birthweight was protective against Kpn infection (OR: 0.858 [95% CI: 0.745–0.987] per 100g increase), while longer hospital stay was associated with increased odds of infection (OR: 1.148 [95% CI: 1.012– 1.1.303] per additional day). Most infecting isolates (34/38 [89%]) were resistant to first- and second-line antimicrobial agents, but all were sensitive to meropenem and 33/36 [92%] to amikacin.

Low birthweight infants with prolonged hospital stay were at greatest risk of Kpn infections that were typically resistant to WHO first- and second-line antimicrobial therapy. These infants should be prioritised for antibiotics that have the potential to be life-saving. The overlapping and evolving nature of these risk factors makes it difficult to design a simple tool to support empiric initiation of meropenem. Neonates critically ill with Kpn sepsis cannot, however, afford to wait for blood culture confirmation before receiving effective treatment. This highlights the need for empiric decision-making frameworks that allow rapid initiation of effective therapy in high-risk neonates.

Objetivo
Identificar las características clínicas de la autoinmunidad neurológica CASPR2 en niños y profundizar en la comprensión de la enfermedad para desarrollar estrategias de diagnóstico y tratamiento.

Métodos
Se realizó un análisis retrospectivo multicéntrico y una observación prospectiva de la autoinmunidad CASPR2 en los últimos 7 años.

Resultados
Veintiséis pacientes positivos para anti-CASPR2 fueron incluidos en este estudio, incluyendo 25 con positividad sérica y 3 con positividad en líquido cefalorraquídeo (LCR) (2 pacientes con positividad tanto sérica como en LCR); 3 pacientes fueron co-positivos con anticuerpos anti- NMDAR y 1 fue co-positivo con anticuerpos anti-GABABR. Once pacientes (6 que se manifestaron con epilepsia refractaria, 4 que se manifestaron con anomalías psicoconductuales y 1 acompañado de germinoma) presentaron títulos bajos de anticuerpos, exámenes de RM/EEG/LCR relativamente normales y mala respuesta a la inmunoterapia y por lo tanto fueron considerados falsos positivos (42,3%). Quince pacientes fueron diagnosticados con autoinmunidad neurológica relacionada con anti-CASPR2: 10 con encefalitis autoinmune (incluyendo 2 que se manifestaron como síndrome de Morvan y 1 cuya condición fue secundaria a encefalitis japonesa), 4 con características clínicas típicas de encefalopatía autoinmune y 1 con cerebelitis autoinmune. Los síntomas más comunes incluyeron trastornos de la conciencia (10/15), fiebre (8/15), síntomas psicológicos/conductas anormales (8/15), trastornos del sueño (8/15), convulsiones (7/15), trastornos del movimiento (5/15), síntomas autonómicos (5/15), hiperexcitabilidad del nervio periférico/neuromiotonía (5/14) y debilidad/hemiplejía (4/15). La resonancia magnética cerebral reveló anomalías en 10 pacientes (66,7%). Los sitios más comunes de lesiones fueron la corteza cerebral (6/15, ampliamente distribuida en los lóbulos frontal, parietal, occipital y temporal) y el tálamo (5/15). Los registros de electroencefalografía (EEG) revelaron un fondo de ondas lentas en 13 pacientes (86,7%). Cinco de 15 pacientes mostraron leucocitos elevados en el LCR, y 4 de 15 pacientes mostraron niveles elevados de proteínas en el LCR. Trece pacientes recibieron inmunoterapia (se adoptó rituximab en 2 casos) y se recuperaron bien. Dos pacientes recibieron tratamiento sintomático, pero la recuperación fue lenta y estuvo acompañada de anomalías emocionales y retraso en el desarrollo.

Conclusiones
La enfermedad por autoanticuerpos anti-CASPR2 no es muy rara en niños y puede presentarse en la infancia. Los fenotipos clínicos más comunes de la autoinmunidad relacionada con CASPR2 fueron el fenotipo de encefalitis, incluyendo el síndrome de Morvan y la ataxia cerebelosa. También informamos por primera vez un caso de encefalitis autoinmune secundaria a encefalitis japonesa. El pronóstico de esta enfermedad es bueno y el rituximab puede utilizarse en pacientes que responden mal a la inmunoterapia convencional. La alta tasa de falsos positivos de anti-CASPR2 en la epilepsia refractaria y las anomalías psicoconductuales requieren mayor investigación.

Objective
To identify the clinical features of CASPR2 neurological autoimmunity in children and to strengthen the understanding of the disease for developing diagnosis and treatment strategies.

Methods
A multicenter retrospective analysis and prospective observation of CASPR2 autoimmunity in the past 7 years was conducted.

Results
Twenty-six anti-CASPR2-positive patients were enrolled in this study, including 25 with serum positivity and 3 with cerebrospinal fluid (CSF) positivity (2 patients with both serum and CSF positivity); 3 patients were co-positive with anti-NMDAR antibody and 1 was co-positive with anti- GABABR antibody. Eleven patients (6 manifesting with refractory epilepsy, 4 manifesting with psychobehavioral abnormalities and 1 accompanied by germinoma) presented with low antibody titers, relatively normal MRI/EEG/CSF examinations, and poor response to immunotherapy and were thus considered false positives (42.3%). Fifteen patients were diagnosed with anti- CASPR2-related neurological autoimmunity: 10 with autoimmune encephalitis (including 2 manifested as Morvan syndrome and 1 whose condition was secondary to Japanese encephalitis), 4 with typical clinical features of autoimmune encephalopathy, and 1 with autoimmune cerebellitis. The most common symptoms included disorders of consciousness (10/15), fever (8/15), psychological symptoms/abnormal behaviors (8/15), sleep disorders (8/15), seizures (7/15), movement disorders (5/15), autonomic symptoms (5/15), peripheral nerve hyperexcitability/neuromyotonia (5/14) and weakness/hemiplegia (4/15). Brain MRI revealed abnormalities in 10 patients (66.7%). The most common sites of lesions were the cerebral cortex (6/15, widely distributed across the frontal, parietal, occipital and temporal lobes) and thalamus (5/15). Electroencephalography (EEG) recordings revealed a slow wave background in 13 patients (86.7%). Five of 15 patients showed elevated WBCs in the CSF, and 4 of 15 patients showed elevated protein levels in the CSF. Thirteen patients received immunotherapy (rituximab was used in 2 cases) and recovered well. Two patients received symptomatic treatment, and the recovery was slow and accompanied by emotional abnormalities and developmental delay.

Conclusions
CASPR2 autoantibody disease is not very rare in children and can occur in infancy. The most common clinical phenotypes of CASPR2-related autoimmunity were the encephalitis phenotype, including Morvan syndrome and cerebellar ataxia. We also first reported a case of autoimmune encephalitis secondary to Japanese encephalitis. The prognosis of this disease is good, and rituximab can be used in patients who respond poorly to conventional immunotherapy. The high false-positive rate of anti-CASPR2 in refractory epilepsy and the psychobehavioral abnormalities need to be explored further.

La esmulación magnéca transcraneal repeva (EMTr) es un tratamiento prometedor para el trastorno por consumo de tabaco (TCT). Si bien a nivel grupal la esmulación acva supera al placebo, a nivel individual existe variabilidad en la respuesta clínica. Los factores conductuales y neurobiológicos que diferencian a quienes responden a la EMTr de quienes no lo hacen aún no están claros. Para explorar los factores individuales que influyen en las respuestas agudas a la EMTr, N = 60 parcipantes humanos recibieron una sesión de EMTr en la corteza prefrontal dorsolateral (CPDL) y en una región de control (corteza visual; V5) en orden aleatorio. Completaron evaluaciones conductuales y neuroimágenes antes y después de las sesiones de EMTr. Las hipótesis que involucraban predictores conductuales y de neuroimagen de la respuesta se registraron previamente a la finalización de la recopilación de datos. La rTMS aplicada al DLPFC produjo reducciones significavas en el deseo de fumar autoinformado en comparación con la rTMS aplicada a una región cerebral de control ( p = 0,0006) y los parcipantes se clasificaron como n = 38 respondedores y n = 22 no respondedores. Los respondedores consumieron significavamente más cigarrillos al día ( M = 11,441) en comparación con los no respondedores ( M = 7,952), informaron niveles más altos de antojo de cigarrillos ( d = 1,059) y absnencia de nicona más severa ( d = 0,803) antes de la rTMS. DETERMINANTS OF REPETITIVE TRANSCRANIAL MAGNETIC STIMULATION EFFICACY IN TOBACCO USE DISORDER: A PRE-REGISTERED STUDY

Repeve Transcranial Magnec Smulaon (rTMS) is a promising treatment for tobacco use disorder (TUD). Although at a group level, acve smulaon outperforms sham, at an individual level, variability exists in clinical response. The behavioral and neurobiological factors that differenate those who respond to rTMS from those who do not remain unclear. To explore individual factors that influence acute responses to rTMS, N = 60 human parcipants received one session of rTMS to the dorsolateral prefrontal cortex (DLPFC) and to a control region (visual cortex; V5) in a randomized order. They completed behavioral assessments and neuroimaging before and aer rTMS sessions. Hypotheses involving behavioral and neuroimaging predictors of response were pre-registered prior to compleon of data collecon. rTMS to the DLPFC led to significant reducons in self- reported cigaree craving compared with rTMS to a control brain region (p = 0.0006) and parcipants were classified as n = 38 responders and n = 22 nonresponders. Responders used significantly more cigarees per day (M = 11.441) compared to nonresponders (M = 7.952), reported higher levels of cigaree craving (d = 1.059), and more severe nicone withdrawal (d = 0.803) prior to rTMS. Neuroimaging analyses based on preregistered hypotheses indicated that DLPFC-frontoparietal and insula whole-brain funconal connecvity did not differ significantly between responders and nonresponders. However, exploratory analyses revealed that responders had reduced pre-rTMS funconal connecvity between the insula and nucleus accumbens, precuneus, and occipital pole. These findings suggest that response to rTMS for TUD is associated with greater baseline cigaree consumpon, craving, and withdrawal, in addion to disnct funconal connecvity paerns related to salience, reward, and self-referenal processes, providing candidate behavioral and neural markers for personalized rTMS intervenons for TUD.
